Dear Residents and Resident Representatives at Wesley Health Care Center:
As a nursing home resident or representative, our facility is required to inform you that all residents have certain rights and protections under Federal and state law that help ensure you get the care and services you need. You have the right to be informed, make your own decisions, and have your personal information kept private.
The nursing home must tell you about these rights and explain them in writing in a language you understand. They must also explain in writing how you should act and what you’re responsible for while you’re in the nursing home. This must be done before or at the time you’re admitted, as well as during your stay.
At these links below, you will find a biannual notification handout that we are providing to all of our residents regarding those specific rights. You will find both a summary handout as well as the Bill of Rights Handbook for your review.

Grievance Notification Process at Wesley Health Care Center
At Wesley Health Care Center in Saratoga Springs, New York, the comfort, safety, health, and happiness of our residents are our top priorities. To continually enhance the quality of life within our facility, we have a dedicated Grievance Officer to ensure that residents can easily voice their suggestions, concerns, and comments without fear of reprisal.
We fully support and promote residents’ rights, including the right to a straightforward and responsive grievance procedure. If you have any concerns about the quality of services, care, or the confidentiality of your medical information, you can file a grievance by contacting our Grievance Officer at 518-691-1487 or via email at grievanceofficer@thewesleycommunity.org.
Additionally, you may file a complaint with BHNNY by calling the hotline at 518-262-4369. Once a grievance is received, it will be promptly referred to the appropriate department for resolution. A response detailing the resolution will be provided within 21 days, if possible.
